Case Manager Resume Example

A case manager resume is read for caseload, population and outcomes: how many clients you carry, who they are, and what changed because you coordinated their care. Hiring supervisors in hospitals, health plans and social service agencies want those three facts before they read anything else.

The Sample resume below belongs to a case manager with six years in community health and social services, currently carrying a mixed caseload of adults with chronic illness and housing instability. Every bullet names a population, an action and a measured change: readmissions, missed appointments, benefits secured, days to placement. That pattern works whether you hold a social work license, a nursing license or a human services degree, and it shows the judgment a supervisor is hiring for.

Match the setting in the posting. A hospital discharge role wants transitions of care and payer knowledge; a county or nonprofit role wants benefits, housing and crisis work. The healthcare hub collects related clinical examples, the registered nurse example shows the clinical side of care coordination, and the Civic template suits public-sector applications.

How the resume changes with experience

The same case manager resume, written at three levels: a summary and three bullets for each.

Entry level

Zero to two years, or a first role in the title

Human services graduate with a year-long field placement in a community health agency, trained in intake, needs assessment and resource referral for adults and families.

  • Completed intake interviews for about ten new clients a week during a field placement, entering assessments into the agency's records system.
  • Built a one-page directory of local food, transit and utility assistance that the intake team still hands to clients.
  • Shadowed senior case managers on home visits and wrote progress notes they signed off without revision.

Mid level

Three to seven years, owns a process or a workload

Case manager with four years coordinating medical, behavioral health and benefits services for adults in a nonprofit program, owning a full caseload from intake to discharge.

  • Carry a caseload of 40 clients and meet every monthly contact requirement in the program's quality review.
  • Cut missed specialist appointments on my caseload by arranging rides and reminder calls the day before.
  • Represent the program at the county housing coordination meeting and track shared referrals.

Senior

Eight years or more, leads people, budgets or programs

Senior case manager and team lead with ten years in care coordination, supervising case managers and designing the workflows a program is audited against.

  • Supervise eight case managers, review their caseloads weekly and handle escalated client situations.
  • Redesigned the intake and assessment workflow, shortening the time from referral to first visit by a week.
  • Negotiated data-sharing agreements with two hospitals so discharges reach the team the same day.

Skills to put on the case manager resume

Hard skills

Assessment and care planning
Supervisors want proof you can turn an intake interview into a written plan with goals, services and dates, since that plan drives every visit after it.
Benefits and entitlement programs
Knowing Medicaid, SNAP, SSI and local assistance rules lets you remove the barriers that most often derail a client's care.
Transitions of care
Hospital and health plan roles are measured on readmissions, and coordinating discharge follow-up is how case managers move that number.
Documentation in an electronic record
Charts are audited for funding and compliance, so accurate, timely notes are a hiring requirement rather than a nice extra.
Crisis intervention and safety planning
Most caseloads include clients in crisis, and interviewers ask how you handled one to judge your composure.
Community resource knowledge
A strong local network of landlords, clinics and agencies is what turns a referral into a service the client actually receives.

Soft skills

Motivational interviewing
Clients change behavior when they set the goals, and this technique shows you can guide without taking over.
Boundaries and self-care
Supervisors look for case managers who stay steady over years of hard stories, since turnover hurts clients most.
Collaboration across agencies
Care is split across many systems, and the case manager who works well with each of them gets results faster.

Mistakes that cost case manager interviews

  1. Listing duties instead of a caseload

Provided case management services tells a supervisor nothing new. State the caseload size, the population and the setting in the first line of each role, then show what you achieved with them.

  2. No outcomes at all

Case management has measurable results: housing placements, benefits approved, readmissions avoided, appointments kept. Pick the two or three your program tracked and put your numbers against them.

  3. Hiding your license

An LMSW, LCSW, RN or CCM is often a hard requirement. Put it in the headline or right under your name as well as in the certifications section, so a screener sees it in the first seconds.

  4. Client details that break confidentiality

Stories help in the interview, not on the page. Describe populations and patterns, never an identifiable client, and keep any example general enough that no one could be recognized.

  5. One resume for every setting

Hospital, insurer and nonprofit roles use different words for the same work. Rewrite your bullets for each application using the posting's terms, such as utilization review, transitions of care or rapid rehousing.

Questions about the case manager resume

What should a case manager put in a resume summary?

Your license or degree, years of experience, the populations you serve and your usual caseload size, plus one outcome you are proud of. Three sentences is enough; the bullets carry the detail.

Do I need a social work license to be a case manager?

Not always. Many community programs hire with a bachelor's in human services or a related field, while hospitals and health plans often ask for an LMSW, LCSW or RN. Read the posting and lead with whichever credential it names.

How do I show results as a case manager?

Use the measures your program reported: placements, approvals, readmissions, missed appointments, time to first visit. Even without exact figures you can say what improved and over what period.

How long should a case manager resume be?

One page for most applicants, two if you have a decade of experience or supervisory roles. Keep licenses, caseload and outcomes on the first page.

How do I move from a nonprofit into hospital case management?

Emphasize medical coordination, discharge work and insurance knowledge from your current role, and name the electronic record systems you have used. A nursing or clinical social work license makes the move much easier.

Every application goes out fitting the job.
