Case managers are hired by very different employers, from hospital utilization teams to health plans and community agencies, but the screen is much the same everywhere: which license you hold, how many clients you carry and in what setting, and what your coordination changed. This template places a short Licenses and Certifications block straight after the summary, so a supervisor checking for an LMSW, an RN or a CCM finds it without scrolling.

Each experience heading has room for the program and the organization, and the meta line asks for your caseload size. The bullet slots follow one pattern: who the clients were, what you arranged for them, and a measure your program tracked, such as readmissions, housing placements or authorizations processed. Skills are grouped as assessment and planning, systems and documentation, and languages, and education closes the page.

Replace every bracket with your own detail and keep client information general enough that no one could be recognized. The case manager resume example shows a completed version.

How to fill in each section

  1. Summary: population, setting and caseload

Open with the clients you serve rather than with traits. A reader at a health plan wants to know whether you have worked with Medicaid members, and a shelter wants to know whether you have worked with people coming in off the street. Give your caseload as a number, name two or three kinds of partners you coordinate with, and finish with the program the posting describes. Rewrite this paragraph for each setting you apply to.

  2. Licenses and Certifications: before the work history

Many case manager postings require a particular license, and some accept several. List the one you hold first, with its state and expiry, so a screener can confirm it in one look. A CCM or nursing case management certification follows with the year. Short trainings belong here only when the posting names them; otherwise keep the block to licenses and formal certifications.

  3. Experience: caseload, coordination, outcome

Put the caseload in the meta line so every role can be compared at a glance. The first bullet describes the population and how quickly you assessed new referrals. The second shows coordination: the agencies, benefits or providers you connected clients with, and how often it worked. The third uses a figure your program already reported, so you are not estimating. Keep any client example general enough to protect privacy.

  4. Skills: methods and systems by name

Group the clinical methods you use, such as assessment, discharge planning or utilization review, apart from the systems you document in. Name the electronic record or case management software and any medical necessity criteria you apply, because hospital and payer roles test for both. Benefits knowledge, such as Medicaid eligibility rules in your state, deserves its own mention.

  5. Education: degree and field, briefly

List the degree that qualifies you for your license, usually an MSW or a nursing degree, with the school and year. Add field placements only if you are new to case management, one line each with the setting and population. Experienced case managers can drop the undergraduate degree when the page is full.

Questions about the case manager resume template

How do I list a caseload that changed over time?

Give the typical range for the period, such as caseload of 35 to 45, rather than your busiest month. If it changed because of a promotion or a move to a new program, list the two titles separately under the same employer, each with its own number.

Can a nurse use this template for RN case management?

Yes. Put your RN license and any nursing case management certification in the first block, and write the summary around the clinical side: utilization review, transitions of care, the payer or hospital setting. Your bedside years fit in the second experience slot as one role with two or three bullets. Nurses still applying for bedside jobs should use the registered nurse resume template.

What should I leave off a case manager resume?

Client names, identifying stories and event photos, even with permission. Leave off duties every case manager has, such as maintained case notes, unless you can add a standard you met. Remove expired licenses and old trainings the posting does not ask for.

Should I include volunteer work?

Include it when it shows the population or the skill the posting asks for, such as crisis line shifts, refugee resettlement or work in a recovery program. One line each, with hours or length of service. Drop it once your paid experience covers the same ground.

Is it free?

Yes. The Word files and the Google Docs copy are free to download, edit and use for any application. There is no signup and no email.

Will it read correctly in an applicant tracking system?

The layout is one column with standard headings and real text: no tables, text boxes, icons, columns or images, which are the parts parsers most often misread. Keep it that way as you edit.

Word or Google Docs?

Use whichever you edit in. The Word file opens in Microsoft Word, Pages and LibreOffice. The Google Docs link asks you to make a copy in your own Drive; the original stays blank for the next person. Send the posting the format it asks for, usually .docx or PDF.
